i would like to nominate Luke Foster.
17 year old youth player (son of a work mate) who made the bench for the first time one tuesday night early 90's, came on for a few mins near the end, got tackled, ligament damage never played again.
Worth a mention?

Named as the number 14 away to Cray Wanderers in a 4-0 win in February 1993.  coolsmiley
